Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did
not furnish employment and a place of employment which were free from
recognized
hazards that were causing or likely to cause death or serious physical
harm to employees
in that employees were exposed to hazards of electrocution:
a.Along highway 464B, employees were replacing 14,400-volt lightning
arresters with 7,200-volt lightning arresters on Blichton feeder lines four
and five, consisting of three 7,200 volts phases each, had not ensured that
the automatic-reclosing devices were tagged or made inoperative. This
violation occurred on or about March 12, 1997.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to ensure that the automatic reclosure devices have been made
inoperative.
Permits and tags must be installed on the oil circuit reclosurer or other
circuit devices
where work is being done on energized parts.
IN THE ALTERNATIVE
29 CFR 1926.417(a):  Controls that were to be deactivated during the
course of work on
energized or deenergized equipment or circuits were not tagged:
a.Along highway 464B, employees were replacing 14,400-volt lightning
arresters with 7,200-volt lightning arresters on Blichton feeder lines four
and five, consisting of three 7,200 volts phases each, had not ensured that
the automatic-reclosing devices were tagged or made inoperative. This
violation occurred on or about March 12, 1997.
