Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
(a)  On or about February 15, 1991, employees were exposed
to crushing injuries in the event of sudden movement of 20' x 2'
diameter tar covered pipe while off loading them from makeshift
transporters (utility tractor) in vicinity of SW 134 Avenue and
Kendall Drive, Miami, FL.
Among other methods one feasible and acceptable method to correct
this is:
1. Provide flat bed trailer equipped with stanchions and
sockets
2. Employ the use of adequate wedges (4" to 6") to prevent
load from shifting
3. Use mechanical means to off load pipe
4. Use lashings to roll pipe off
5. Use trailer designed for hauling pipe
6. Protect an adequate amount of the roadway for workmen
with barricades and flagmen
7. Discharge the load in proximity to where it was to be
used
