free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
In the strip room employees were exposed to crushing injuries and
possible chemical burns from the hazards related to lifting the cover
on sodium hydroxide tank, to place parts into tank, with improperly
rigged lifting equipment on the cover. The crew on the dip tank used
in the strip room and a counterbalance connected to the heavy cover
by means of chain connected leg a nut, bolt and washers. The cover
could fall causing crushing injuries or chemical burns.
One acceptable mean of abatement would be to connect the chain to the
cover by means of clevice and pin.
