Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
Being struck by dies for Press Brakes 1 and 2 that were not secured.
The hold down bolts to secure the dies were not in place as the holes
were stripped in the dies used in Press Brake 1 and 2 in the machine
department.
Among other methods, feasible and acceptable abatement methods to
correct this hazard would be to:
1) Re-bore the stripped holes and replace bolts to insure the dies
are secured in the press brakes;
2) To establish a formal diesetting procedure which includes training
of operators, and what to do if the safeguard is damaged, missing,
inoperative, or unable to provide adequate protection as stipulated
in ANSI B11.3-1982, paragraph 6.5.2

General-duty citation text

Section 5(a)(1) of the Occupational Safety adn Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) In the machine department, employees were required to lift loads
that weighed 10,490 lbs. on the 5 ton Michigan crane. Employees were
exposed to being struck by the load of steel, causing broken bones
and/or death.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to break down all loads over the load capacity
of the crane and adhere to a procedure such as ANSI B30.11-1980
paragraph 11-3.2.1
b) Possible death in that the upper travel limit switch was not tried
out under no load at the begining of each operator's shift on the
5 ton Michigan pendant controlled crane. Among other methods,
