Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
The hazards of fractures, internal injuries, lacerations and
concussions while using a come-a-long with cable and hooks to
apply tension to a rolled cone fabricated unit to align its
circumference to a closed circle before welding. The hazard
was caused when the eye pad weld failed and the cable, hook,
and pad eye recoiled into an employee's pelvic area, causing
him to fall 4 to 5 feet backward and against the cone on or about
July 9, 1990.
Among other methods, one or more feasible and acceptable abatement
methods to correct this hazard are but not limited to:
1) Grind the red oxide primer paint off of the steel area to be welded
to insure a good weld bond.
2) Use the proper welding amperage to match that required for the linear
footage of electrode cable to be used. The proper amperage is also
important in cases where two dissimilar thicknesses exist and
amperage should be matched to the thicker material. In additon, the
diameter of the electrode shall be such as to relate to the thickness
of the material.
3) Refer to the welding manual for the proper selection of the electro
rods to be used for the special project would yield the best results.
