Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
(a)  An improperly designed free-standing steel stack at the
Golden Aluminum Company plant site at 14555 Old Corpus Christi Road,
San Antonio, Texas, on or about March 7, 1991.  The ASMF/ANSI Steel
Stack Standard, STS-1-1986, and recognized industry practice was not
followed.
Among other methods, one feasible and acceptable method to
correct this hazazard is to follow the design criteria specified in
ASMF/ANSI Steel Stack Standard STS-1-1986.

failed to provide detailed field assembly instructions for the stack
erection.
Among other methods, one feasible and acceptable abatement method
to correct this  hazard is to provide adequate instructions to ensure
the proper field erection of steel stacks.

(a)  The collapse of a free-standing steel stack at the Golden
Aluminum Company plant site at 14555 Old Corpus Christi Road, San
Antonio, Texas, on or about March 7, 1991.  The employer failed to
ensure the integrity of the field welds by adequate inspections and
non-destructive testing of the welds by recognized methods, such as
ultrasonic, radiographic testing, etc.
Among other methods, one feasible and acceptable abatement method
to correct this hazard is to conduct inspections and testing to ensure
the integrity of the field welds.
