The employer is not limited to the abatement methods suggested by OSHA; the methods explained are general and may not be effective in all cases; and the employer is responsible for selecting and carrying out an effective abatement method. Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) A fall hazard, of approximately sixty-two (62) feet above the metal drilling rig floor existed when an employee(s) was permitted to ride the elevators which were attached to the traveling block and there were no emergency conditions. One feasible and acceptable abatement method among others to correct the hazard is to assure employees riding the elevators shall wear a safety belt with appropriate lanyard and safety attachment and adjusted to prevent a fall of more than five (5) feet such as specified by API RP54 First Edition January 1981, page 11, paragraph 4.8.3(b). This condition existed on or about August 6, 1992.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) The hazards of being struck by an unsecured pressurized flowline containing pressure of approximately four thousand (4,000) psi,located at Arrow Driling Company Inc., Rig #3, on the J.H. Ewing Lease #7. One feasible and acceptable abatement method among others to correct the hazard is to secure the flowline to the ground. Essential elements of this procedure would include: (1) Secure or anchor the flowline at the end of the flowline and at intermediate intervals along the line each fifteen to twenty feet. (2) Where the mud hose is connected the flowline shall be anchored to the ground by means of chains, steel stakes or other adequate restraint on both ends of the mud hose. (3) That employees shall not be permitted to step across and/or on a pressurized flowline.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) The hazard of being restricted in escape from the monkeyboard in the event of fire or explosion. This occurred onor about August 6, 1992, on Arrow Drilling Company, Inc., Rig #3. Among other methods one feasible and acceptable abatement method to correct this hazard would be installation of a properly rigged and secured emergency descent device or other satisfactory means of egress, such as specified in section 4.7, paragraph 4.7.1 of API RP54, First Edition 1981, page 11. Essential elements of this procedure would include: (1) staking out a securely anchored one-half (1/2) wire rope escape line attached to the derrick or mast so as to provide a ready and convenient means of escape from the derrickman's working platform. (2) the escape line route is to be kept clean of obstructions. %% (3) a safety device is to be provided to permit fast emergency exit from the derrickman's working platform to a safe place. (4) the tension on the escape line is to be periodically checked and adjusted to assure a safe landing and to allow the derrickman to stop twenty (20) to twenty-five (25) feet from the anchor point. (5) the ground anchor point of the escape line is to be located a minimum lateral distance from the derrick or mast equal to the height above to the point where the escape line is fastened to the derrick or mast.
