General-duty citation text

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fall hazards while working from elevated forks of a
fork-lift and or empty
elevated pallets without utilizing any means of fall protection:
Cooler men were using fork-lifts to elevate themselves to freezer storage
racks which were
approximately 14 feet off the floor(coolers 2 and 3) without using fall
protection, man-baskets
and or guard-rail systems.
Among other feasible and acceptable methods to correct this hazard would
be to;
1) Follow guidelines listed in the American Society of Mechanical
Engineers(ASME) B56.1-
2000, Safety Standard for Powered Industrial Trucks, section
4.17-Elevating
Personnel-with a
High Lift Truck designed to elevate personnel, and section
7.36-Platforms-Elevating-provide the
restraining means such as railing, chains, cable, body belt(s) with
lanyard(s), or deceleration
devices, that are in place and properly used.
2) Follow guidelines listed in the American National Standards Institute
(ANSI) A64.1,
Requirements for Fixed Industrial Stairs, sections 3.1-Where Fixed Stairs
Are Required-for
access to elevations which requires regular travel between levels.
Abatement certification and abatement documentation is required for this
violation.  The
documentation should include written verification of abatement, applicable
measurements
or monitoring results, and photographs or videos which you believe will be
helpful.  The
abatement certification sheet is enclosed with the citations(s).
