Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burns, falls, and explosion hazards due to the escape system not being in place: a) On or about April 14, 2005 and at times prior, Grey Wolf Drilling Co. Lp did not ensure that employees were protected from fall, fire, and explosion hazards due to the geronimo line not being installed at the monkeyboard. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").-Certification

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to boiler explosions due to the boiler not being properly inspected: a) On or about April 14, 2005 and at times prior, Grey Wolf Drilling Co. Lp did not ensure that employees were protected from explosion hazards. The boiler used to supply steam heat to the rig had not been inspected as required. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").-Certification
