Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to falling loads due to worn and improperly attached running
lines:
a)  On or about April 14, 2005 and at times prior, Kimsey Casing Service
Inc. did not
ensure that employees were protected from running line hazards.  The
employees did
work under and in line with the trough used to lower casing pipe.  The
line was severely
worn in several spots and the connections of the wire rope were not done
according to
the requirements of the wire rope industry.
