Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards due to job made repairs and fabrication not being tested: a) On or about April 18, 2007 and at times prior, Elenburg Exploaration Inc did not ensure that employees were protected from struck-by hazards. 1. A piece of round stock was bent into a hook and used for lifting pipe, it had no rating assigned to it. 2. A tong hanger handle had broken and it was field welded, no record of welder certification or weld inspetion was available. The second tong hanger handle on the rig was twisted severely. 3. Crosby clamps were not properly installed in that the distance and torque as required by the manufacturer was not followed. 4. Hoist line was not removed from service when it had more than the allowed broken wires and bird caging. One feasible and acceptable method of abatement would be to purchase new handles and hooks that meet manufacturers specifications and carry ratings of strength and use. Critical repairs and fabrication of parts that are exposed to extreme loads and torque should be followed by the parts and repairs being tested for strength. Inspections as required by API RP 54 9.6 and replacement of all parts that do not meet the listed criteria. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").
