Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the struck-by hazards:
(a) On or about February 15, 2008 and at times prior, employees were
exposed to struck by
hazards while using hoists without daily to monthly inspections. The hoist
adjacent to
machine #12 had no stated load rating and the safety latch was broken on
the hook.  The
Hercules hoist in the Tool Room had a damaged safety latch on the hook.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is to:
(a) Determine and marked the rated load of the hoist adjacent to machine
#12 on the hoist or
load block. Per ASME B30.21-1994  Paragraph 21-1.1.1(a).
(b) Maintenance of hooks per ASME B30.10-1999  Paragraph 10-1.2.3(c). A
hook latch,
when required, that is inoperative shall be repaired or replaced.
Maintenance of hooks per
ASME B30.10-1999  Paragraph 10-1.2.3(d). A hook with a latch that does not
close the
throat opening shall be removed from service or moused until the latch is
replaced or
repaired.
(c) Perform frequent inspections at daily to monthly intervals as per ASME
B30.16-1993
Paragraph 16-2.1.2(c) on items such as:
(1) Hooks in accordance with ASME B30.10, Frequent Inspection.
(2) Hook latches, if used, for proper operation.
(d) Perform periodic inspections at monthly to yearly intervals as per
ASME B30.16-1993
Paragraph 16-2.1.3(d), which includes the requirements specified in
paragraph 16-2.1.2(c)
plus:  (1)  Supporting structure and trolley if used shall be inspected,
for continued
ability to support the imposed loads.
(e)  Written, dated and signed inspection reports and records should be
made yearly on
critical items such as hooks, chains and supporting structure. Dated
inspection reports and
records shall be maintained at time intervals per ASME B30.16-1993,
Paragraph 16-
2.1.1(b)(2).  Records should readily available.
