Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) The hazard of inadvertent overpressurization of a tire on the buffing machine and subsequent explosion on or about July 19, 1991 and at times prior thereto, during tire buffing operations. Among other methods, the feasible and acceptable abatement methods to correct this hazard include: (1) Install a pressure relief valve on the tire buffing machine to ensure the manufacturer's stated 15 psi limit on tire inflation is not exceeded. (2) Limit the in line pressure to the tire buffing machine to 15 psi. (3) Install an in line pressure gauge visible to the operator. (4) Ensure all repairs on tire buffing machines and other equipment is performed by qualified personnel in accordance with the manufacturer's direction. (5) Train employees on the safe operation of machinery and ensure safe work practices are always used through regular job site inspections.
