Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being killed when an inadequately anchored roof blew off: (a) The south side of the new banquet room at Templin's Resort Hotel - On or about January 20, 1993 and at times prior thereto, the trusses of the newly sheathed roof were not adequately anchored to the supporting structure of the building. Only one partially driven 16d nail was used to attach the southwest ends of each truss. No Simpson H2.5 or H3 and A35 metal ties had been installed prior to sheathing the roof. Employees on the approximately 2448 sq. ft. roof attempting to secure it from wind uplift were killed when the roof blew off during high wind gusts. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS: Follow the "Truss Plate Institute, Commentary and Recommendations For Handling, Installing & Bracing Metal Plate Connected Wood Trusses" as required by specifications outlined in the Templin's Resort Hotel project manual and install the Simpson H2.5 or H3 metal ties (hurricane ties) and the A35 metal ties shown in the blueprints prior to sheathing the roof. The "Truss Plate Institute" states "The installer should provide adequate attachments of the trusses to the supporting structure at all bearing points immediately as the trusses are being installed. Trusses should not be installed without adequate connections to the supporting structure." It also states, "CAUTION: A minimum of two-16d nails nailed in accordance with NDS criteria should be used for each connection." The Simpson metal ties H2.5 and H3 (hurricane ties) are designed to be installed to resist forces in the event of stress due to wind uplift.
