recognized hazards
that were causing or likely to cause death or serious physical harm to

employees were exposed to crushing injuries, in that tag lines and proper
rigging were not
being used, while moving a concrete septic tank:
A) On or about February 20, 2003 and at times prior to, employees were
exposed to
crushing injuries while in the process of moving a concrete septic tank
without using a tag
line and proper rigging methods.
Among other methods, one feasible and acceptable abatement method to
correct this
hazardous condition would be to use tags lines to control the load and to
keep employees out
from under the loads.  Ensure slings are connected to proper lifting
points prior to lift, and
ensure slings are centered to keep the load level and under control.

this item
