Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:  falling from the flatbed truck and being crushed by the
tire of the flatbed truck
while operating in reverse and falling from the flatbed truck to the road
surface whren operating
forward.
Construction site: Saw Mill River Parkway, Northbpound Lane by Route 119.
On or about 08/27/97
a) Employee(s) placing and retrieving traffic cones were exposed to the
hazards associated with
potential injury of falling and being crushed under the the tire of the
unprotected flatbed truck
used to transport employees to and from worksite, while placing and
picking up traffic cones.
AMONG OTHER METHODS ONE FEASIBLE AND ACCEPATABLE ABATEMENT
METHODS TO CORRECT THIS HAZARD ARE:
1) If employees must work from from the bed of the truck, install and use
a restraint system
such as safety belt/lanyard, attached to adequately anchored eyebolts or
other means of
attachment, standard guard rails.
2) Install a truck mounted work platform with appropriate guard rails and
or other restrain
devices.
NOTE: The employer is required to submit abatement certification for this
item in accordance
with 29 CFR 1903.19.
