Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to an explosion hazard/burns from working with combustible
dust that was not
protected with equipment explosion protection and isolation, as well as
ensuring that all tramp
material had been removed upstream of the grinder:
(a)G & H Forty Niners, Incorporated dba Keystone Protein Company;  Feather
Meal Line - The two (2) cyclones and ring dryer were not equipped with
explosion protection, such as deflagration venting and the cyclones were
not
provided with a backflash damper for isolation protection, on or about
October
31, 2007.
(b)G & H Forty Niners, Incorporated dba Keystone Protein Company;  Poultry
Meal
Line - The two (2) screw conveyors were not equipped with isolation
devices,
such as chokes, and the bucket elevator and grinder were not provided with
explosion protection, such as deflagration venting.  The grinder was
equipped
with a magnet, but not all tramp material was removed prior to reaching the
grinder, on or about October 31, 2007.
ABATEMENT NOTE:  AMONG OTHER METHODS, A FEASIBLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS TO ADD DEFLAGRATION VENTING
TO THE CYCLONES, RING DRYER, GRINDER, AND BUCKET ELEVATOR, TO
INSTALL CHOKES ON THE SCREW CONVEYORS AND BACKFLASH DAMPERS
ON THE CYCLONES, AND TO INSTALL A METAL SEPARATOR UPSTREAM OF
THE GRINDER.
All proposed control measures shall be approved for each particular use by
a
competent
engineer or other technically qualified person.  Ninety (90) day progress
reports are
required during the abatement period.
*STEP 1
Submit to the Area Director a written, detailed plan of abatement
outlining a schedule
for the implementation of engineering and/or administrative measures to
control
employee exposure to hazardous substances as referenced in this citation.
This plan shall include, at a minimum, target dates for the following
actions which must
be consistent with the abatement dates required by this citation:
1.Evaluation of engineering/administrative control options.
2.Selection of optimum control methods and completion of design.
3.Procurement, installation and operation of selected control measures; and
4.Acceptance or modification/redesign of controls.
*STEP 2
Abatement shall have been completed by the implementation of feasible
engineering upon
verification of their effectiveness in achieving compliance.
