What this inspection record means

OSHA opens inspections for many reasons — routine scheduling under a national or local emphasis program, an employee complaint or referral, or a follow-up after a reported injury. Opening or conducting an inspection is not itself an allegation or a finding that this employer broke any rule; any findings appear as the citations listed below, and citations can be contested, reduced, or withdrawn.

Abate by
Sep 30, 1996
Penalty

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to being struck and or crushed by the elevator and to
potential fire hazards within
the elevator car and they were not properly trained in the recognition of
these hazards.
Employees using, operating, and working on this elevator were bypassing
the inside safety gate
and operating the car, opening the outside safety gates by reaching
through openings to open the
doors from floor levels, and operating the elevator car with the inside
car lights dysfunctional
and using a cigarette lighter as substitute lighting. One feasible method
to correct this hazard is
to supply proper training to those employees using the elevator by a
qualified person and require
that all employees who operate the elevator be licensed operators.
A) LOCATION: 123 FIRST ST
THOSE EMPLOYEES REQUIRED TO USE, OPERATE, AND WORK ON THE FREIGHT
ELEVATOR, WERE NOT PROPERLY TRAINED IN THE RECOGNITION AND
AVOIDANCE OF UNSAFE CONDITIONS RELATIVE TO THE ELEVATOR.

5(a)(1)

Willful Gravity 10 2 instances 4 exposed
Issued
Sep 25, 1996
Abate by
Sep 30, 1996
Penalty
Initial $49,000 · Current $24,500 Reduced
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to being crushed or caught by a moving elevator when vision
panels on the first
and third floors were missing, leaving openings into the hoistway of the
elevator. A feasible
method to correct this hazard, among others, is to install the proper
safety glass windows in the
openings.
LOCATION: 123 FIRST ST, FIRST AND THIRD FLOORS
THE OUTSIDE ELEVATOR DOORS ON THESE FLOOR LEVELS, DID NOT PREVENT
AN EMPLOYEE FROM HAVING ANY PART OF THEIR BODY WITHIN THE
HOISTWAY. THE VISION PANELS IN THE DOORS WERE MISSING, LEAVING
OPENINGS LARGE ENOUGH FOR SOMEONE TO PLACE THEIR ARM OR HEAD
THROUGH THEM, INTO THE HOISTWAY OF THE ELEVATOR.
