Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazards created by inadequately maintained equipment: FRONT OF BUILDING, BIN AREA: THE COMPANY DID NOT ADEQUATELY MAINTAIN THE BRUSH BANDIT MODEL 200+, SERIAL NUMBER 004512, ACCORDING TO THE MANUFACTURER'S RECOMMENDATIONS AND DID NOT PROVIDE THE LEVEL OF TRAINING TO OPERATORS AS RECOMMENDED BY THE MANUFACTURER. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD IS TO: 1.IMPLEMENT A PREVENTATIVE MAINTENANCE PROGRAM IN ACCORDANCE WITH THE MANUFACTURER'S RECOMMENDATIONS. 2.PERFORM ALL SERVICE AND MAINTENANCE, SUCH AS, BUT NOT LIMITED TO, KNIFE CHANGES, IN ACCORDANCE WITH THE MANUFACTURER'S RECOMMENDATIONS. 3.ADEQUATELY TRAIN AND SUPERVISE ALL EQUIPMENT OPERATORS TO A LEVEL THAT MEETS THE MANUFACTURER'S RECOMMENDATIONS.
