explosion hazard because
methods were not utilized to contain combustible, organic dusts created by
manufacturing,
handling, and processing operations from becoming airborne and combining
with an ignition
source:
(a)Mixing Area - The mixing machine operation was not maintained under
continuous suction, thus allowing the escape of dust during normal
operations, on
or about April 22, 2006.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable abatement
method to correct these hazards is to comply with National Fire Protection
Association
(NFPA) 654 "Standard for the Prevention of FIre and Dust Explosions from
the
Manufacturing, Processing, and Handling of Combustible Particulate
Solids".  This
includes the following:
(a)Install an appropriate type of dust collection system per Section 4.1
of the
NFPA 654.
(b)Install an appropriate type of dust collector outside of the building
per
Section 3.13.1.1 of NFPA 654.
