Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: fall hazards: (a)East Side of 200 Innovation Boulevard - Employee was not tied off while riding a Sky Genie 32 feet to the roof area while carrying a two (2) by six (6) piece of wood for framing of the roof vents, on or about September 16, 2003. ABATEMENT NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard includes using personal fall arrest system, with the lanyard attached to the boom or basket of the personnel lift, as outlined in the American National Standards Institute safety standard for Boom-Supported Elevating Work Platforms (ANSI/SIA A92.5 - 1992). VERIFICATION REQUIRED
