Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to potential crushing injuries while working under the
suspended roof assemblies:
(a)Roof Section - Employee(s) were working under a roof assembly that was
suspended above the floor by chains that were not approved for overhead
lifting
and by a spreader bar that was connected to two (2) hoists.  The spreader
bar was
not marked with the rated load and the weight of the spreader bar.  Two
(2) "A-
frame" secondary support units were in place under the suspended roof
assembly,
but these units were also not marked with their rated load capacity.  The
employer did not have engineering documentation to demonstrate that these
devices, along with the overhead beam that supported the hoists, were
adequately
designed and there were no inspection records maintained to ensure the
integrity
of the lifting assembly , on or about July 17, 2003.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS TO BUILD THE ROOF SYSTEMS DIRECTLY
ON THE HOUSING UNIT, OR TO HAVE AN ENGINEERING STUDY PERFORMED ON
THE ENTIRE LIFTING SYSTEM AND SECONDARY SUPPORTS TO ENSURE THIS
DESIGN CAN ADEQUATELY SUPPORT THE LOAD.  REPLACE THE CHAINS WITH A
GRADE 80 ALLOY CHAIN THAT IS DESIGNED FOR OVERHEAD LIFTING.  CONDUCT
PERIODIC INSPECTIONS ON THE LIFTING DEVICES AS PER ANSI B30.16 - 1973,
OVERHEAD HOISTS, AND ASME B30.20-1999, BELOW-THE-HOOK LIFTING DEVICES.
THE NAMEPLATE FOR THE SPREADER BAR SHOULD HAVE THE
MANUFACTURER'S NAME AND ADDRESS, THE SERIAL NUMBER, THE LIFTER
WEIGHT, AND THE RATED LOAD.

were exposed to severe burn injuries from the potential dust explosion
hazard that could have
resulted from, or have been exacerbated by, the location of the dust
collector inside of the
building, the lack of deflagration venting on the dust collector, the lack
of magnetic separators
or equivalent means to prevent the entry of tramp metal into the dust
collection system, the lack
of designated smoking areas,  and not ensuring that the filtered air was
not recirculated back into
the plant:
(a)Homes by Keystone, Incorporated; Dust Collector Room - The Torit dust
collector that was used to collect combustible wood dust was located
inside of the
building in a separate room, but the dust collector was not equipped with
deflagration venting, on or about July 17, 2003.
(b)Homes by Keystone, Incorporated - Magnetic separators or similar
devices were
not provided to prevent the entry of tramp metal into the dust collection
system
from the floor and equipment intakes located in the Cabinet Shop, on row
about
July 17, 2003.
(c)Homes by Keystone, Incorporated - Smoking was not restricted to
designated
areas, on or about July 17, 2003.
(d)Homes by Keystone, Incorporated - Adequate measures were not in place to
ensure that the filtered air was not recycled back into the building, on
or about
July 25, 2003
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THESE HAZARDS INCLUDES FOLLOWING NFPA 664,
STANDARD FOR PREVENTION OF FIRES AND EXPLOSIONS IN WOOD PROCESSING
AND WOODWORKING FACILITIES, 1998 EDITION, INCLUDING BUT NOT LIMITED
TO:  LOCATE THE DUST COLLECTOR OUTSIDE OF THE BUILDING, INSTALL
DEFLAGRATION VENTING IN THE DUST COLLECTOR, INSTALL MAGNETIC
SEPARATORS OR OTHER SIMILAR DEVICES TO PREVENT TRAMP METAL FROM
ENTERING THE DUST COLLECTION SYSTEM, DESIGNATE SPECIFIC SAFE AREAS
FOR SMOKING, AND ENSURE THAT THE FILTERED AIR IS NOT RECYCLED BACK
INTO THE BUILDING.
