recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to severe burn injuries from the potential dust explosion
hazard that could have
resulted from, or could have been exacerbated by, the location of the dust
collector inside of the
building, the lack of any isolation devices in the ductwork, and the lack
of  a self-closing fire
door where required:
(a)New York Wire Company, Incorporated - The dust collector that was used
to
collect combustible dust was located inside of the Mixing Room and was not
provided with explosion protection, on or about July 15, 2003.
(b)New York Wire Company, Incorporated - Isolation devices were not
provided to
prevent the deflagration propagation between the dust collector and the
mixer, on
or about July 15, 2003.
(c)New York Wire Company, Incorporated - A fire door was not provided at
the
entrance leading into the Mixing Room from the Coating Department, on or
about
July 15, 2003.
"AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THESE HAZARDS INCLUDES, FOLLOWING NFPA 654,
STANDARD FOR THE PREVENTION OF FIRE AND DUST EXPLOSIONS FROM THE
MANUFACTURING, PROCESSING, AND HANDLING OF COMBUSTIBLE PARTICLE
SOLIDS, 1997 EDITION, INCLUDING, BUT NOT LIMITED TO:  LOCATE THE DUST
COLLECTOR OUTSIDE OF THE BUILDING AND PROVIDE SUITABLE EXPLOSION
PROTECTION;  INSTALL AN ISOLATION DEVICE, SUCH AS A FLAME FRONT
DIVERTER OR EXTINGUISHING SYSTEM, IN THE DUCTWORK;  AND INSTALL FIRE
DOORS AT ALL OPENINGS THAT LEAD TO ANOTHER ROOM.
