an aluminum platform was not secured:
(a) Line 1, Metal Roofs Department - An open sided aluminum platform that
measured
approximately twenty (20) inches in width by twenty (20) feet in length
was not secured
against movement/displacement.  An employee fell approximately eight (8)
feet, ten (10)
inches from the unsecured/sliding platform and suffered fatal injuries, on
or about February
18, 2002.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO
CORRECT THIS HAZARD IS TO SECURE THE ALUMINUM PLATFORM FROM SLIDING.
