General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees were exposed to: (a) Employees were exposed to a potential crushing hazard while working near the manually operated manlifts in the main elevator and the Shannard elevator due to the lack of guardrails around the base of each manlift. Among other methods, one feasible and acceptable method to correct this hazard would be to install a guardrail with an entrance gate at each manlift. (b) Employees were exposed to a potential fall hazard while operating the manually operated manlifts due to the lack of a midrail on the car enclosure. Among other methods, one feasible and acceptable means of abatement would be to install an additional chain which would serve as a midrail on each manlift.
