Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees:
(a)  At the basement level, a door or gate was missing from the hoistway
opening,
exposing employees to injury from inadvertent physical contact with the
moving
material platform.
(b)  At the first floor and intermediate dock level, the hoistway gates
were not
equipped with mechanical locks and electric contacts, exposing employees
to injury
from falls and inadvertent physical contact with the moving parts of the
equipment.
Feasible methods of abatement, among others, would be:
1.  To install a door or gate at the basement hoistway opening which
prevents
physical contact with the moving parts of the equipment, as required by
ANSI
B20.11976,
Rule 6.21.2.1.
2.  To install mechanical locks and electric contacts to each hoistway
gate, so that
they can be opened only when the platform has stopped at that level and
the platform
cannot be moved until they are closed, as required by ANSI B20.1-1976, Rule
6.21.2.2.6,
