(a) On or about August 1, 1996, at the Dakota Gasification Company
construction
site, an employee of Swanson and Youngdale, a subcontractor under the
control
of
Dakota Gasification Company, was exposed to a potential fall hazard while
working
from an elevated pallet on the extensible boom forklift.  The pallet used
as a platform
was not secured to the lifting carriage or to the forks to prevent it from
sliding off the
forklift.  Among other methods, one feasible and acceptable method to
correct this
hazard would be to secure the platform to the carriage or forklift.
