Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to amputation injuries from accidental operation of thepress
brake where the actuating pedal mechanismm was not protected to
prevent unintended operation from falling or moving objects or by
accidently stepping on the foot treadle.
Aong other methods, a feasible and useful means to correct this
hazard is to install a foot treadle guard.
(a) Vacuum forming area; the Wysong Press Brake (model
1548, serial #PB36-266) did not have its foot treadle
guarded exposing employees to injury.
