Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to crashing injuries from failure of a brake on
a powered metal press during feeding or removing parts from presses
while they were guarded at the point of operation by a two hand control,
presence sensing device, type B gate or movable barrier guard:
a)   Sizing Department - The Gasbare sizing presses (number
1 and 2) equipped with a two hand control device.
Among other methods, feasible and acceptable methods of abatement
include:
a)   Installation of a brake monitor system with an approved
control designed, constructed, and arranged to monitor
the effectiveness of the powdered metal press braking
system.
b)   Installation of point of operation devices, such as
pull back restraints, that would not require a brake
monitor.
