furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck by drill rod tongs during the
process of breaking
down drill rods from the boring machine. The procedure being used was not
in accordance with
the procedure specified in the Operator's Manual.
At the jobsite approximately 1/2 mile east of Saratoga Road west of
Morris, IL,
employees engaged in breaking apart drill rods were not following
manufacturer's
procedures specified in the Operators Manual.
