Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not provide a place of employment that was free from recognized hazards in that employees were exposed to having their head and torso entrapped between the truck and ground in the event of tip over. Employees operating forklifts with serial numbers 404283A and 1058394A did not use the seat belts provided. Among feasible methods to abate this hazard is to comply with the American National Standard Institute (ANSI) Safety Standard for Low Lift and High Lift Trucks, ANSI B56.1- 1988, Section 5.3.19 and wear seat belts at all times operating the truck. DOCUMENTATION AND CERTIFICATION REQUIRED FOR THIS ITEM.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not provide a place of employment that was free from recognized hazards that were likely to cause death or serious physical harm to employees in that employees were exposed to being struck by falling dies due to failure of the lifting devices (eyebolts and hoist rings) : In the tool room, eyebolts and hoist rings used to lift dies were not marked with the rated load. Among feasible methods to abate this condition is to: Comply with the American National Standards Institute (ANSI) Safety Standard for Below the Hook Lifting Devices, ANSI B30.20-1993, Section 1.2.1 and affix permanent markings as to the manufacturer, serial number, lifter weight, and rated load. CERTIFICATION AND DOCUMENTATION REQUIRED FOR THIS ITEM.
