General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a potentially uncontrolled chemical reaction that could result in toxic gases and liquids or impact injuries: (a) Cyanide storage area: On or about 2/15/00, employees were exposed to the hazard of a potential chemical reaction evolving poisonous gases from storage of incompatible chemicals in the same area. Four 50 kilogram containers of solid potassium cyanide and one container of solid sodium cyanide were stored approximately 25 feet from an open 55 gallon drum of sulfuric acid, with no berm between them. Among other methods, useful abatement methods include storing chemicals in separate areas, and ensuring that any chemicals received from suppliers are immediately separated from incompatible chemicals.le
