Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious harm to employees in that employees were exposed to crushing injuries from rolling truck(s) during loading and unloading operations: Loading Area - The brakes of tractor trailer truck # 9339 that was being loaded and unloaded by a forklift driver were not set and did not prevent the truck from rolling. One feasible means of abatement would be a check list of things to do in a conspicuous area of the cab of the truck, that the truck driver should do before he exits the truck after docking and one listing should state SET BRAKES!!!us
