Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck or crushed:
Truck #3136 - Modifications were made to the 1995 McNeilus rear loading
truck, by
installing a winch.  The company did not have documentation stating the
person who
made the modification, date, and a statement attesting to the compliance
of ANSI Z245.1

Among other methods, one feasible method of abatement is to contact an
certified engineer who
has the ability to determine the requirements and limits set forth in ANSI
Z245.1-1992 4.3
entitled "American National Standard for Refuse Collection, Processing,
and Disposal
Equipment-Mobile Refuse Collection and Compaction Equipment-Safety
Requirements"

were exposed to the hazard of being struck or crushed:
Truck #3136 - On or about 9/29/03, a driver was struck by a hook and cable
system
when an employee hooked it to a bar in order to realign the compactor when
the bar
bent.  The cable slipped off and the hook struck the employee in the jaw.
Employees
had been previously trained in hooking to one-point connected parts during
this type of
operation.
Among other methods, one feasible method of abatement is to allow hooking
only to two-point
connected parts and/or backing up the truck again in order to realign.
Training employees in
these, or other acceptable methods per ANSI Z245.1-1999 9.3 entitled
"American National
Standard for Refuse Collection, Processing, and Disposal Equipment-Mobile
Refuse Collection
and Compaction Equipment-Safety Requirements".
