Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees
in that
employees were exposed to falls from elevated tower(s) and personnel
platform(s) due to
improperly erected and/or maintained hoisting equipment:
In the following instances hoisting equipment was not properly erected
and/or
maintained:
a)Tower 6 - The hoist line used to raise or lower employees was not
equipped with a swivel to prevent rotation of employees.
b)Tower 6 - The personnel load capacity and material capacity of the
lifting system was not posted at the hoist operator station.
c)Tower 6 - The lifting capacity and drawings of the gin pole were not
available at the job site.
d)Tower 6 - The hoist was not level.
e)Tower 6 - The recommended operating speeds and special hazard
warnings or instructions were not posted on the hoist.
f)Tower 6 - Hoist exhaust pipes were not guarded.
g)Tower 6 - An accessible fire extinguisher was not available at the hoist
operator station.
h)Tower 6 - The operating manual or manufacturer's information for the
hoist was not maintained at the job site.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to follow the applicable ANSI and manufactures information.
