Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to falling and crushing hazards:
(a)  No positive method was provided to assure that all hoistway
doors were closed before the elevator car on the cable elevator
would move exposing employees on the first and second floor to
a fall hazard and the employees in the basement to a crushing
hazard.  Among other methods, one feasible and acceptable
abatement method to correct this hazard is to install electrical
interlocks on the door that would prevent the elevator car from
moving until all hoistway doors were closed.
