Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were causing or likely
to cause death or serious physical harm to employees.  The employer had
not instituted a policy and
procedures for conducting periodic and regular inspections of horizontal
injection molding machines to insure
that safeguards are in safe operating condition and a proper adjustment:
(a)  8/4/93,On the injection molding machines ncluding three Reed 200 ton
Model TES, two Reed 200 ton
Model TG, two Reed 300 ton Model TES, two Reed 400 ton Model TES,
one Reed 200 ton Model TD,
two Newbury 200 ton Model 200, one Newbury 150 ton Model RS,two
Newbury 75 ton Model RS, two
Beloit 200 ton Model RS, one Van Dorn 110 ton Model RS6, one New
Britian 175 ton, no Model
Number.
One feasible means of abatement to follow the safety systems
maintenance program as published in the
Operating Instructions and Service Information for Newbury Imperial
Series:
Safety systems maintenance
At the start of each 8 hour shift and after each die change,
check the operation of the safety gate
limit switches, the hydraulic safety gate valve and the
mechanical drop-bar safety, operated by the
safety gate.  Do not operate the machine unless all safety
features are working properly.
While checking, stay clear of moving machine members. Turn the
semi-manual-auto selector switch
to "auto". Then with motor running, machine at the idle
position, the safety gate open and the drop
-bar flag in position to prevent the machine from closing,
check out the systems as follows:
1.  Depress the hydraulic safty gate valve. Holding this
depresses, close the gate.  Clamp should not
move.
2.  Be sure the drop-bar flag drops in front of the stud
whenthe
gate is opened.  The distance
between the stud and the flag should be approximately 1/4 inch.
3.  With safety gate depressing the hydraulic interlock valve,
close the gate until it is off the
interlock valve, but not making the safety gate limit switch,
1-LS.  Clamp should not move.
4.  To check the rear guard safety, remove the rear gate to
make the switch inoperative.  Then close
the front safety gate.  Clamp should not move.
Always be sure that the hydraulic interlock is being depressed
completely and the drop-bar system
is properly adjusted.
