Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that:
a)On or about 3/29/94. Employees were exposed to the hazard of being struck
by and/or crushed by a 13,000 lb concrete form which was being lifted by a
P & H
15 ton crane.  The form was suspended by a two-legged wire rope bridal
sling which
was attached to a 3/8 inch grade 70 carbon steel chain. The chain was used
for
overhead lifting was not in accordance with the manufacturer's
recommendations and
the requirements of ANSI B30.9 1990, section 9-1.1.  The chain
manufacturer, in its
product manual, lists a working load limit of 6,600 lbs. and does not
recommend
grade 70 chain for overhead lifting.
Among other methods, a feasible and acceptable methods to correct this
hazard
is to
use an alloy steel chain which is rated for the maximum intended load to
be lifted.
