Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
On or about 10/11/91, the employer did not furnish employment and a
place of employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to employees
in that employees were exposed to the hazard of serious crushing
injuries from the dropping of rotary hydraulic automotive lifts while
employees were working under vehicles on the lifts.  Of the eleven
lifts in the service area, five had locking devices which did not
engage.  The locking devices were inoperable due to excessive ac-
cumulations of dirt, rust and oil in the latch mechanism.  Also, the
locking devices on these lifts could only be engaged when the lift is
fully extended:
a- Some feasible and reasonable abatement methods are, among
others:
1- When working under a lift that is not fully extended and
locked, use found jack stands or equivalent means for a
positive means of support.
2- Instruct and train employees in the proper use of the
manufacturer's mechanical locking devices and safe operating
instructions for the lifts.
3- Conduct required maintenance procedures described by the
manufacturer.
4- Conduct the recommended inspections of the lift by the
manufacturer to ensure that the mechanical locking devices
and all other components are in safe operating condition.
Remove defective lifts from service until they are rendered
safe for use.
5- Post and maintain permanently on display the
manufacturer's safe lifting instructions in the lift area.
6- Follow the requirements of ANSI B153.1 and the Automotive
Lift Institute publications, ALI-SM89-1 (Lifting it Right).
Note:  The lifts with inoperable locking devices were located
in Bays 3, 4, 6, 7 and 8.
