Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
On or about 11/25/91, the employer did not furnish employment and a
place of employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to employees
in that employees were exposed to the hazard(s) of being struck by
vehicles suspended on the rotary automotive lifts:
a- Employee(s) were exposed to hazard(s) of being struck by
vehicles suspended on the rotary automotive lifts.  There
are four lifts at the garage.  Three are single post type with
safety devices which are designed to automatically engage only
at full extension.  Only two lifts are currently operable.
The fourth lift is a dual post type which is utlilized in
conjunction with the wheel alignment machine.  The single post
lifts in Bay 2 and 3 were observed to have safety locks which
were rendered ineffectual because rust prevented the latches
from automatically working.
Feasible and acceptable methods, among others, to correct the
hazard is to:
1- Support vehicles by the use of four jack stands or equivalent
means when the lifts are not used at full extension and
employees are working under them.
2- Instruct employees on the proper use and limitations of the
lifts as per the manufacturer's recommendations and ensuirng
these recommendations are followed; updating and repeating
training as needed.
3- Develop and maintain a preventive maintenance program as
per the manufacturer's information, including, but not limited
to, keeping the locking latches free and oiled and checking
the oil level.  Remove defective lifts from service until
repairs can be completed.
4- Post permanently on display the manufacturer's safe lifting
instructions in the lift area.
