Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
a.  Employees were exposed to multiple traumatic injuries,
extensive burns and death from fire and/or explosion  on/or
before September 26, 1991.
The employer did not determine that the chemical plant processing
equipment, piping and associated safety devices met acceptable
safe engineering designs and specifications.

The employer did not develop and implement safe operating pro-
cedures for the FM-1- FA-1 chemical process inclusive with the
flammable and combustible storage tank cleaning procedures.
"AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT-EACH OF THE TWO CONDITIONS ARE:
"SAFE ENGINEERING DESIGNS AND SPECIFICATIONS"
(a)  Develop process equipment piping and instrumentation
diagrams (P&ID'S) to ensure that all safety features are applies
to the equipment and system(s) as follows:
1.  construction
2.  material(s)
3.  electrical
4.  sources of ignition
5.  pressure relief system(s)
6.  normal and emergency venting
7.  bonding and grounding
8.  normal and emergency procedure(s)
-and-
"DEVELOP AND IMPLEMENT SAFE OPERATING PROCEDURES FOR THE
FM-1 AND FA-1 CHEMICAL PROCESS"
(a)  Develop and institute written standard operating
procedures for the FM-1 and FA-1 process relating to reactor(s)
3 and 4 (to include normal and abnormal procedures).
(b)  Develop and institute standard operating procedures
for the cleaning process of flammable/combustible liquid storage
tanks.
(c)  Conduct training for all operators and mechanics on:
1.  process design system(s) and P & ID(s)
2.  process standard operating procedure(s).
