Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and lacerating injuries: %% a) Paste Mold - G furnace - frequent and periodic inspections of 5 ton pendant controlled monorail hoists being utilized to lift double cans of cullet were not performed. On or about 5/17/91. %% b) Blow Shop - Y tank - frequent and periodic inspections of a pendant controlled monorail hoist (Capacity not posted) being utilized to lift individual cans of cullet were not performed. On or about 5/17/91. %% Among other methods, one feasible and acceptable method to correct hazard is to establish and adhere to an inspection program and other requirements as referenced in ANSI B30-11, Chapter 11-2. %%
