Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a) The hazard of being struck by a moving elevator.  On or about 1/29/98,
at the fifth
floor service elevator landing, the glass was missing from the vision
panel on the left (as
viewed from the landing) section of the three-section horizontally sliding
hoistway door,
allowing the placement of body parts through the door into the hoistway.
Employees
approached the opening and looked through and/or put their heads into the
opening in
order to determine which floor the elevator was on.
Among other methods, one feasable and acceptable abatement method to
correct this
hazard is to:
a) Install protective grills made from stainless or galvanized steel, not
less than
0.0568 in. (1.4427mm) in thickness into each vision panel in the hoistway
doors in
accordance with ASME A17.3-1996, Safety Code for Existing Elevators and
Escalators,
Section 2.6.3(i)  OR  modify hoistway door vision panels to comply with
ASME A17.3-
1996, Safety Code for Existing Elevators and Escalators, Sections 2.6.3(a)
through
2.6.3(h).
