Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: A CRUSHING HAZARD CREATED BY THE SUDDEN AND UNPREDICTABLE DECENT OF VEHICLES ELEVATED BY HYDRAULIC AUTOMOTIVE LIFTS. a) SERVICE AREA, BAY #1, ON OR ABOUT 09/13/93, SINGLE PISTON LIFT - MECHANICAL LOCKING DEVICE WAS INOPERATIVE AND NO PERMANENT MARKING ON LIFT TO INDICATE RATED LOAD CAPACITY. b) SERVICE AREA, BAY #2, ON OR ABOUT 09/13/93, SINGLE PISTON LIFT - MECHANICAL LOCKING DEVICE WAS MISSING AND NO PERMANENT MARKING ON LIFT OT INDICATE THE RATED LOAD CAPACITY. c) SERVICE AREA, BAY #3, ON OR ABOUT 09/13/93, SINGLE PISTON LIFT - MECHANICAL LOCKING DEVICE WAS INOPERATIVE AND NO PERMANENT MARKING ON THE LIFT TO INDICATE THE RATED LOAD CAPACITY. d) SERVICE AREA, BAY #4, ON OR ABOUT 09/13/93, HUNTER ALIGNMENT RACK WITH SURFACE MOUNTED LIFT - LIFT LOCKING DEVICE WAS MISSING AND EQUALIZING CABLES WERE MISSING CAUSING LIFT WITH VEHICLE MOUNTED TO DROP APPROXIMATELY 6-INCHES TO 12-INCHES AND NO PERMANENT MARKING ON LIFT TO INDICATE RATED LOAD CAPACITY. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS TO.... 1. INSTALL AND/OR REPAIR MECHANICAL LOCKING DEVICES 2. INSTALL PERMANENT MARKING ON LIFTS TO SHOW RATED LOAD CAPACITY. 3. INSTRUCT ALL EMPLOYEES IN THE SAFE USE AND OPERATION OF THE AUTOMOTIVE LIFTS AND THEIR SAFETY DEVICES AND ENSURE THEIR ROUTINE USE. 4. ESTABLISH A PERIODIC INSPECTION PROCEDURE AS RECOMMENDED BY THE MANUFACTURER TO ENSURE THE SAFE OPERATON OF THE LIFT. 5. FOLLOW THE REQUIREMENTS OF ANSI/ALI B153.1-1990, AUTOMATIC LIFT INSTITUTE PUBLICATION ALISM89-1 AND NIOSH PUBLICATION NO. 77-22.
