Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to the hazard of being struck by a high pressure hose or having
large quantities of water puncture the skin, flesh or organs which
could result in death:
a.   BRIDGE DECK: Rt. 695 Northbound to I-690 Westbound
BIN #1093280 Solvay, NY, 13209;  (1) employee using
a high pressure water lance (6,000 PSi operating
pressure) connected to an NLB Corp. model 1045D high
pressure pump (3/4" plungers) serial number 092260-1
(mac. pressure 10,000 PSi).  The hose connecting the
lance to the pump was not marked as to its pressure
rating.  Among other methods one feasible method to
reduce this hazard is to follow NLB Corp's Operation
and Maintenance Manual which requires:
1)   A safety and equipment check list be used (para
2.3 B attached).
2)   Hoses marked to show maximum permissible
operating
pressure (para 4.8).
3)   Testing before use and periodically throughout
use (para 5.3)
