employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
a)  West Side of Office Building, 299 Cherry Hill Road
Employee(s) were using a Rigid 300 Electric Pipe Threader, serial
number 7600650, which was provided with a constant pressure foot
threadle control switch.  The Rigid 300's foot threadle however, was
kept in the on position due to the placement of a 3 inch diameter
plumbing elbow which had been placed between the threadles housing
and foot control treadle.  Employee(s) were exposed to the hazard of
being caught in the equipment
