Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) West Side of Office Building, 299 Cherry Hill Road Employee(s) were using a Rigid 300 Electric Pipe Threader, serial number 7600650, which was provided with a constant pressure foot threadle control switch. The Rigid 300's foot threadle however, was kept in the on position due to the placement of a 3 inch diameter plumbing elbow which had been placed between the threadles housing and foot control treadle. Employee(s) were exposed to the hazard of being caught in the equipment Violation occurred on or about 1/13/92
