Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of falling with the scaffold and or being
struck by the falling scaffold
in the event of primary support line failure:
a)Southwest corner - West line 10:
1. Employees were working on a multi-stage suspended scaffold that was not
equipped with
additional independent support lines attached to an independent suspension
member, on or about
9/19/94.
2. Employee working from the lower stage of the multi-stage suspended
scaffold was not tied
off on to the top scaffold structure, on or about 9/19/94.
"Among other methods, one feasible and acceptable abatement method to
correct
this hazard is
to:
1.  Equip multi-stage suspension scaffolds with additional independent
support lines equal in
number to the number of points supported, and equivalent in strength of
the suspension ropes,
and rigged to support the scaffold in the event the suspension rope(s)
fail.
2.  Independent support lines and suspension ropes shall not be attached
to the same points of
anchorage.
3.  Supports for stages shall be attached directly to the support stirrup
and not to any other
stage.
4.  Have employees working from lower stages of the multi-stage suspension
scaffold tie off to
the top scaffold structure.
