Section 88.4 - Code of Iowa (1999) 88.4 - The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazards of the work piece not being properly clamped causing incorrect control manipulation: (a)Assembly Area - On 10/31/00, an employee was drilling/tapping existing holes on the bottom of a die shoe. The blocks holding the work piece (bottom die shoe) were not secured to the bed of the drill press allowing the blocks to shift causing the work to slide, and possibly fall, resulting in an injury to the operator as he was trying to hold the die shoe to prevent it from falling. Among other methods, one feasible and acceptable abatement method to correct the hazard is to secure all work in place prior to performing operations as referenced in the Accident Prevention Manual for Industrial Operations, Engineering and Technology, Ninth Edition, Chapter 10 (Drills), pg. 241.
