Section 88.4 - Code of Iowa (2001) 88.4: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of multiple operators operating machines with only one operator having control: (a)North Building, Manufacturing Area, Hendley & Whittemore Steel Roller Machine - On 10/16/01 three employees were performing work while operating a 12 foot steel roller machine (#14550). There were three employees operating the machine performing production operations. Two of those employees were working on the operating side (in-feed position) while the other employee was on the discharge side (out-feed position/stacker). Only one operator had control of the machine although the production being run required multiple operators in the danger zone. Among other methods, one feasible and acceptable abatement method to correct the hazard is to provide controls for all operators working in the danger zone as referenced in ANSI B11.1, 1971, Safety Requirements for the Construction, Care, and Use of Mechanical Power Presses, Sections 5.3.3, 5.3.4 and 5.3.5, and 29 CFR 1910.217(c)(3)(vii)(a). y y J p\
