Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to a fall hazard of approximately 15 feet to the floor below:
(a)  Crown Center, Kansas City, Missouri - Employees working in the
Purple Level Central Plant were required to climb over pipes 15
feet or more above floor level to operate valves and were not
provided any means of fall protection.
While ultimate responsibility for correcting the hazard rests
with the employer, given superior knowledge of the workplace,
one feasible and effective means of correcting this hazard,
among others, would be to:
1.  Modify the valve control locations so that all valves could
be reached from an elevating work platform or a ladder
without necessitating that employees climb onto pipes.
Essential elements would be to:
a.  Modity the valve control locations so tha all valves
could be reached from an elevating work platform or
ladder.
b.  Provide chain-type valve handle extensions.
